Community Inclusion Worker with Passport Hours - Willowbridge Community Services
Full time (24 hours per week in Respite, 5 hours flexible Passport, 3 hours reception

About The Role:
We are looking for a dynamic Community Inclusion Worker, who will assist adults and children with a developmental disability by participating in meaningful social inclusion activities in the community, and ensuring the provision of trauma and attachment informed services is welcoming and safe. This role will
work Monday- Friday with a rotational 4 Day Work Week (4DWW) in the Respite program and 5 hours for Passport Supports. This role also includes one evening working at Reception until 7:30pm (either Tuesday or Wednesday) with a start of 11am that day.
In this position you are required to support participants with a variety of social, recreational and life skills activities based on the individual’s interests and goals using a person-centered approach. Working under the guidance of the program supervisor, you will assist individuals living with developmental disabilities
to live a good life, develop friendships, feel connected to community, develop a socially valued role, support individuals to independently set goals, develop skills and participate in leisure and social activities. You will also support individuals to access public transportation, or when necessary, transport
clients in your own vehicle. The Passport supports will be working with individuals eligible for passport funding to assist them in connecting to community and managing their funds.
Anticipated start date: August 5, 2025
Qualifications
- College diploma in a social services field, or equivalent combination of education and experience.
- Experience in office administration would be preferred.
- CPR/First Aid Certificate (or able to obtain one).
- Safety Care Certification (or able to obtain one).
- Valid driver’s license and access to a reliable, insured vehicle.
- Strong commitment to supporting individuals in actively participating in their community.
- Able to support individuals in their daily living activities.
- Ability to help plan and carry out activities for individuals and groups.
- Experience with individuals with a developmental disability, dual diagnosis or complex and/or challenging behaviours.
- Flexibility to work weekends/evenings if participants require those hours.
- Experience working with children (ie. Camp settings)
- Excellent communication and interpersonal skills.
- Proficiency in computer literacy and ability to use Microsoft Office including Outlook, Word and Excel.
- Must be able to perform manual (1 or 2 person assist) and /or operate mechanical Hoyer lift.
- Knowledge of Passport programs would be an asset.
