37.5 hours per week, primarily Monday to Friday, days with the flexibility to work some evenings.
Duties
The Addiction and Mental Health Case Manager will work within the Brantford Rapid Access Addiction Medicine (RAAM) Clinic’s multi-disciplinary team. Your role will focus on providing support to individuals 16 + who experience addictions and concurrent disorders. You will provide support on site, in person at the clinic and through the Digital Front Door. The Digital Front Door is a virtual clinic that enables individuals seeking care for substance use concerns to virtually access care within the RAAM clinic.
As an Addiction and Mental Health Case Manager, you will:
- Collaborate effectively with the RAAM multi-disciplinary team and other community partners
- Develop relationships and rapport through person-centered interactions
- Respond to referrals in a timely manner
- Conduct in person and virtual intakes and provide case management support as needed
- Work with clients in developing a support and/or safety plan based on their case management needs
- Utilize harm reduction, motivational interviewing and brief solution-focused approaches
- Facilitate warm connections to ongoing addiction, mental health, and other health and social services
Qualifications
- Have a college diploma in Social Services; a university degree in Social Sciences is preferred
- Have a minimum of 2 years’ experience working with individuals who use substances, those with mental health issues, and other vulnerable populations
- Have demonstrated ability to establish client rapport, assess need, develop support plans, mitigate crisis, and navigate health and social service systems to facilitate client connection
- Demonstrate skill in working collaboratively with service delivery partners
- Have access to reliable transportation
- Be able to manage frequent exposure to vicarious trauma and occasional exposure to direct trauma
