Casual PT Registered Practical Nurse (RPN) - SOAR Community Services

Duties
SOAR Community Services Brantford has been an established part of the community for more than 55 years, and is accredited for our excellence in serving children, youth, adults, and families.
The Registered Practical Nurse (RPN) provides direct patient care within residential Withdrawal Management and Treatment Services to the full scope of practice for an RPN. Client needs are met by providing screening, intake assessment, withdrawal symptom monitoring using evidence-based protocols, administration of medication, provision of physical care, and contribution to development of a collaborative and comprehensive clinical discharge plan.

The RPN:
• Adheres to CNO Standards of Practice and demonstrates practice competencies
• Conducts in-person and telephone screening of clients prior to admission
• Completes comprehensive client intakes
• Monitors clinical condition, withdrawal symptoms, administers oral and injectable medications and documents client care effectively
• Identifies clients requiring higher level of care and facilitate transfer to, and return from, hospital if required
• Collaborate with interdisciplinary support team members, such as Nurse Practitioner and Withdrawal Management Workers, and external partners (i.e., pharmacy, RAAM medical team, hospital) to ensure safe and effective practice and continuity of care
• Collects specimens such as blood, urine, and blood glucose
• Assists with medication reconciliation and EMR chart maintenance
• Maintains a safe and clean facility

We will collaborate on a training and capacity building plan to grow skill and confidence in working with our client population. We also support ongoing professional development.

Qualifications
• Registration in Good Standing with the College of Nurses of Ontario (CNO)
• Practical Nurse Diploma from a program recognized by CNO
• Ideal candidates will have related clinical experience in an addictions or mental health setting (experience in the management of withdrawal from substances is preferred)
• Demonstrated knowledge of CNO Standards of Practice and commitment to maintaining reflective practice
• Proof of valid First Aid/CPR certification
• Proficiency in MS Office and familiar with databases
• Access to reliable transportation
• Ability to manage frequent exposure to direct and vicarious traumatic stress
• Please also note that the successful candidate will be required to provide a police vulnerable sector check as a condition of employment, however strong consideration is made to the type of offense
